Question: “When working out asset allocation, do financial planners usually start with overall net worth, and then work back from there? Or do they not include everything in the asset allocation calculations? I can find plenty of examples of how to work out asset allocations, just nothing on what should be included when figuring what you have to start with.”

Financial planners usually segregate liquid assets from your net worth. Of course, Net Worth = Assets – Liabilities
Most financial planners will only do asset allocation for investments in the stock and bond markets. They won’t include your home, rental real estate, business, etc.
They will break it down into Large Cap, Mid Cap, Small Cap, International, ST bond, Intermediate Bonds, LT bonds, REITs, sectors, etc.
